1. Acceptance of These Terms

By using https://www.drrizviwoundcare.com (the “site”), you agree to these Terms of Use. If you do not agree, please do not use the site. These Terms apply to every page on the site and to any service we offer through it, except where a separate agreement governs (for example, our patient-portal terms).

2. Who Can Use This Site

The site is intended for adults aged 18 and older, and for parents and legal guardians acting on behalf of a minor. We do not knowingly collect information from children under 13.

3. Permitted Use

You may use the site for personal, non-commercial purposes — to learn about the practice, to read patient education, to schedule a visit, to complete an intake form, or to contact us.

4. Conduct You May Not Engage In

While using the site, you agree not to:

  • Attempt to access any part of the site you are not authorized to access.
  • Interfere with the site’s operation, security, or other users.
  • Scrape, copy, or republish the site’s content for commercial use.
  • Upload viruses, malware, or other harmful code.
  • Impersonate another person or misrepresent your identity.
  • Use the site to harass, threaten, or defame anyone.
  • Use the site to violate any applicable law.

5. Information on the Site Is Not Medical Advice

Content on the site — articles, descriptions of services, frequently-asked questions, and similar information — is for general education and is not medical advice. It is not a substitute for an evaluation, diagnosis, or treatment by a qualified clinician. Do not delay care or change a treatment plan based on something you read on the site.

6. In an Emergency

If you are having a medical emergency, call 911 or go to the nearest emergency department. Do not use the site or contact form to report an emergency.

7. Using the Site Does Not Create a Doctor-Patient Relationship

Reading the site, sending us a message, or completing an intake form does not by itself create a doctor-patient relationship between you and Dr. Hina Rizvi or any other clinician. A formal relationship begins after a documented evaluation, in person or by telemedicine, and our acceptance of you as a patient.

8. About Our Physician and Practice

Dr. Hina Rizvi, M.D, C.W.S is licensed to practice medicine in Texas and is Board Certified by the American Board of Wound Management. The practice operates from 7709 San Jacinto Place, Suite 100, Plano, TX 75024. This disclosure is provided in compliance with the Texas Medical Board’s physician-advertising rules at 22 Texas Administrative Code Chapter 164.

9. Telemedicine

Telemedicine visits are conducted on a secure two-way video and audio platform that complies with HIPAA and with the Texas Medical Board’s telemedicine rules in 22 Texas Administrative Code Chapter 174 and the Texas Occupations Code Chapter 111. You will receive a separate consent form before a telemedicine visit. See our telemedicine page for service details.

10. Privacy and Your Health Information

How we collect, use, and protect your health information is described in our Notice of Privacy Practices. How we comply with civil-rights and language-access requirements is described in our Non-Discrimination & Accessibility Notice. Your right to a written cost estimate when you pay out of pocket is described in our Good Faith Estimate notice.

11. Cookies, Analytics, and Tracking Technologies

The site uses cookies and similar technologies to remember your preferences, keep the site secure, and measure how visitors use it in aggregate. We follow the OCR bulletin on online-tracking technologies and do not use third-party trackers in any way that would disclose your protected health information without your authorization. We do not sell information collected on the site, and we do not use it for cross-context behavioral advertising. Most browsers let you control or disable cookies; doing so may limit some features. 12. Communications You Receive From Us

If you give us your phone number or email, we may contact you about appointments, clinical follow-up, billing, and care reminders. The federal Telephone Consumer Protection Act (TCPA) treats two kinds of communication differently:

  • Treatment-related text messages (appointment reminders, follow-up instructions, post-procedure check-ins) are sent only to numbers you have given us as part of your care. Reply STOP at any time to opt out.
  • Marketing text messages (information about a service you do not currently use, surveys, promotions) are sent only if you give a separate written consent on a form that says you are opting in to marketing texts. You can revoke that consent at any time by replying STOP, in writing, or by calling us.

Standard message and data rates from your carrier may apply. Reply HELP for help, or call 972-491-1200. If you do not wish to receive marketing email, you can unsubscribe using the link in the email; we honor unsubscribe requests within 10 business days as required by the federal CAN-SPAM Act.

13. Patient-Submitted Content

If you post a review, complete an intake form, or send us photographs or a message, you confirm that the information is yours to share. You give us permission to use what you submit for treatment, payment, healthcare operations, and — for reviews and testimonials — for the purpose for which you submitted it. We will not use any identifiable photograph for marketing or education without your written authorization.

14. Intellectual Property

Text, photographs, graphics, and design on the site are the property of Dr. Rizvi Wound Care or are used under license. You may view and print pages for personal, non-commercial use. You may not copy, redistribute, modify, or create derivative works from the site without our written permission.

15. Third-Party Links and Services

The site may link to information published by third parties — for example, the U.S. Department of Health and Human Services, the Texas Medical Board, or insurance carriers. Those websites are governed by their own terms and privacy policies. We do not control them, do not endorse them, and are not responsible for their content.

16. Disclaimer of Warranties

The site is provided as is and as available. We do not warrant that the site will be uninterrupted, error-free, or secure, or that information on the site is complete, accurate, or up to date. Your use of the site is at your own risk.

17. Limitation of Liability

To the maximum extent permitted by Texas law, Dr. Rizvi Wound Care and Dr. Hina Rizvi are not liable for any indirect, incidental, consequential, special, exemplary, or punitive damages arising from your use of the site, even if we have been told that the damages are possible. Nothing in these Terms limits any liability that cannot legally be limited.

18. Indemnification

You agree to defend and hold harmless Dr. Rizvi Wound Care and Dr. Hina Rizvi from any claim arising out of your use of the site or your breach of these Terms.

19. Governing Law and Venue

These Terms are governed by Texas law, without regard to its conflict-of-laws rules. Any dispute arising from your use of the site will be brought in the state or federal courts located in Collin County, Texas.

20. Changes to These Terms

We may update these Terms from time to time. The “Last updated” date at the top of the page reflects the most recent change. Continued use of the site after changes are posted constitutes acceptance of the revised Terms.

21. Severability

If any part of these Terms is found unenforceable, the rest will remain in full effect.

Information you give us as a patient (your medical history, photographs of your wound, billing information for your visit, and so on) is governed by our HIPAA Notice of Privacy Practices. This Website Privacy Notice covers the information we collect when you visit https://www.drrizviwoundcare.com, read pages, fill out a non-clinical form, or accept cookies on this site.

What This Notice Covers

This notice describes how Dr. Rizvi Wound Care (the “practice,” “we,” or “us”) collects and uses information through https://www.drrizviwoundcare.com. It does not cover:

  • Protected health information collected during a clinical visit, telemedicine consultation, or by phone (governed by our HIPAA Notice of Privacy Practices).
  • Information collected by hospitals where Dr. Hina Rizvi holds privileges; their notices apply when you visit them.
  • Information collected by third parties whose services we link to from this site.

Information We Collect

Information You Give Us Directly

When you contact us, request an appointment, or complete an intake form, you give us information such as your name, contact details, reason for visit, and anything you choose to write in a message. If the form collects clinical information, that part is treated under HIPAA and our Notice of Privacy Practices.

Information We Collect Automatically

  • Server logs (IP address, browser type, referring page, time of visit).
  • Approximate region inferred from IP address (state and country, not precise location).
  • Cookies and similar technologies (described in the Cookie Preferences section below).

Analytics and Measurement

If you opt in to analytics, we may use the following tools to measure aggregate use of the site:

  • Google Analytics 4 — aggregate visit and event data, with IP anonymization where supported.
  • Microsoft Clarity — anonymous session recordings and heatmaps to find usability issues.
  • Vercel Analytics — basic page-view counts at the hosting layer.

How We Use Information

  • To respond when you contact us through the site.
  • To schedule appointments and answer billing questions.
  • To keep the site working, fast, and secure.
  • To understand which pages patients find useful and where to make improvements.

How We Share Information

We do not sell information collected on this website. We share information in only a few situations:

  • Cookies: as noted in the Cookie Preferences section below; most session cookies expire when you close your browser, persistent cookies last up to 13 months.

Security

We use HTTPS for every page, modern security headers, and access controls on the systems that power the site. No transmission over the internet is perfectly secure; please do not include sensitive medical information in a non-secure form on this site.

Email and Text-Message Communications

If you give us your email or phone number through the site, we may use them to respond, to schedule a visit, or to follow up on your care. We send marketing email only if you sign up for it, and you can unsubscribe at any time using the link at the bottom of every marketing email.
